Before our consultation, please complete a brief screening questionnaire. This helps me understand your background, why you’re seeking therapy, and how you plan to pay (insurance, out-of-network, or private pay).Once you schedule the consultation through my Google calendar, I will send you the link to fill out the intake form securely through my TherapyNotes EHR system.

After completing the intake form, we’ll have our 15-minute video consultation. During this call, we’ll discuss your needs in more detail and determine the best next steps.
